Empowering Post-Hospice Patients with Care Solutions and Resources
Sometimes, patients who are on hospice experience stabilization of their disease to the point that they no longer meet the Medicare requirements for hospice. Being discharged from hospice can be overwhelming to patients and their family care partners who are faced with continuing care needs.
Wings of Hope designed the Advanced Illness Care (AIC) program to ease the transition from hospice by continuing to provide guidance on care solutions and to connect the family with needed resources.

Advanced Illness Care services
Our AIC program is currently available only to individuals who are being discharged from partnered hospice providers. Members of Wings of Hope's AIC team work together to provide support in the following ways.

Perform continued assessment of patient and family needs

Identify care and support solutions suited to the patient’s unique circumstances

Provide educational materials to help the patient stay healthy and avoid hospitalization
Family Caregiver Education
AIC families are also welcomed to attend Wings of Hope’s education series that provides resources and support to caregivers of family members with life-limiting or disabling conditions. These videos are provided at no cost to both our client families and the public in our service area. Several classes are dedicated to dementia-related care. (Coming Soon!)
